This achievement is obtained during the final boss fight with Killabilly. After the first section, you will be free to move around. Simply cut off as many fingers as possible each time he lays his hand down. Be careful not to lower his health too much by shooting, otherwise you'll kill him before cutting off 20 fingers. Refer to the video below if you need more help.
