Quickest way from level 49-50 is to go to New Jersey trainyard, to where the 'C' in Clinton is on the map. There is a rare innocent there looks like a bum, punch him twice and execute, for 5k respect, then kill yourself for him to respawn. you spawn at the doctor 2 blocks away from him, giving you about 5k a minute endlessly.
