This achievement is complete and pure evil. I'm going to post my methods here and as others make progress we can hopefully come up with the optimal solution. It's assumed that you have already completed one run through the game!

The most important thing you need to know is how to get gems.
Method 1:
You'll get a bonus wave every 5 waves you complete, which, if you complete successfully will give you a free item. This has a chance to give you 5-15 gems.
Method 2:
Gems also drop occasionally. There is a charm you can use to make enemies drop more items, which seems to increase the number of gems they drop as well. With that charm, I was getting 2-5 gems per wave (Grind wave 29/39/49 for the most gems)
Method 3:
Pachinko sometimes gives you gems. Do NOT count on this, but it's a nice bonus. I only used this when it was really desperate and close to an upgrade. It seemed like you have a very small (think tenths of a percent) chance of getting gems each time you get a jackpot.

The next thing to know is what to upgrade:
The only unit you'll need to upgrade is the farmer. You may also upgrade frosty the bowman, but don't use gems on him! The reason for this is that by end game you'll be using your units solely to keep enemies occupied while your skills recharge. Your units have limited health and damage, while your enemies health and damage will continue to rise each wave.
For skills, you'll want to upgrade troop trample. Spend all of your gems upgrading this skill. Every once in a while you may upgrade tornado, as it's the only other skill you'll need really. Troop trample gives you the best damage for your gems and can be upgraded infinitely. Most of your other skills have a maximum level which limits their usefulness. Health, bow damage, and sword damage are not very useful. They only increase by a small amount each level, and you'll be doing minimal damage to your enemies even after spending hundreds of gems!

Finally, strategy
You'll want to use troop trample and tornado as your skills. Each wave consists of your spamming farmers while using your bow to do small amounts of damage to enemies as they pile up against your farmers. You'll want to use troop trample every chance you get. If a wave has flying enemies, try to focus tornado on them, otherwise use it any chance you get as well. Early on, your farmers will be able to take care of most everything except the flying enemies, but near the end you'll be relying on troop trample to break through those really tough enemies.

Many times you'll find your farmers struggling as clerics continually heal the enemies front line. I found the best way to deal with this was to run through the front enemies and use a tornado to wipe out the clerics.

Last, don't be afraid to use revives. I was constantly farming to buy more revives. Some waves are simply too difficult to do without upgrading your skills to insane levels. Reviving one, two, even three or four times is sometimes necessary to pass a level. I found myself stuck especially on wave 49 many times.
