This is one of the secret cheevo's called "Medical malpractice"

This is a story related cheevo and therefore is unmissable.

At the end of episode 1: Low life/ low tech you will be attacked by an assassin and you will enter her memories.

Once the memory sequence is complete you can rewind the memory by rotating the left thumb stick in an anti-clockwise direction and when you see certain object in the sequence go kinda fuzzy pull the thumb stick down and it will give you the option to press B to change the memory.

There is a 2nd cheevo which I advise you get before completing this sequence.

Simply rewind until davids mask flashes and press B to untie his mask which will allow him to talk with olga.

Now you must cause the doctor to kill david. Rewind to the beginning of the memory and allow it to play from the beginning, when the Blue coloured medical thingy flashes pull down on the left thumb stick and then press B to change the endorphin chemical mix.

Continue and follow the same procedure to remove david's arm restraints and his mask. Now the doc will be forced to kill him and the cheevo should pop.
