This will probably be a bit of a confusing achievement for most people. It's not related to leveling up, but to the various upgrades you can find around the map, usually, but not always, marked by a question mark on the map.

There are 8 levels for each of the 6 upgrades, so that makes 48 upgrade items in total. The problem is that not all upgrades will appear on your first playthrough. Or your second. Or third. Rather some will appear on each playthrough, and judging by what people are saying, which ones appear seems to be random. I had all of them by the end of my third playthrough though, so expect to go into New Game+ (by loading your clear game file) at least two times and playing to the end.

Difficulty doesn't seem to matter, and you can choose a new one before every NG+ anyway, so pick whatever you want. I went Hard, Hard, Easy, for the record.

Explore thoroughly. Most upgrades will be marked on the map, but some won't, so you have to keep your eyes open and explore everywhere on every playthrough. For example, the stimpak upgrade to the right of the generator in level 2 is very easily overlooked, as is the grenade upgrade in the room with the crazy replicant on the same level, which can't be revisited once you leave it. The VR Challenge area also holds three upgrades in total; one at the end of the stealth challenge and two on the left side of the main room. They even put a stimpak upgrade in the final boss room on my third run through the game, so nowhere is off limits. Also, sometimes the game will put question marks where there is nothing to be found. A good example is your prison cell on level 5, where the game likes to put a marker after you get the Teleporter. There's nothing there though, and there never will be. If you're unsure, you can always just use your molecular glasses and scour every inch of the environment. Actually you should be doing that regardless.
