The official description for this achievement is:
"Complete the single player game without losing a life."
Now, I happily did this several (4) times and was not rewarded for the achievement and could not for the life of me work out why. The reason is the vague description.
What you actually need to do is complete the Super Challenge Mode without losing a life!
I was playing the Totemball (1 Player), which you can't blame me for doing!
Anyway, work your way through the levels; if you have completed the game before you will hopefully know where the Totem pieces are, just be careful the timer doesn't run out.
If you feel like you're going to run out of time, or you have gone for a jump across water which was just too ambitious, pause the game and return to the Totemball title screen. Go back into Super Challenge Mode and resume your game, this will start you from the beginning of the level with the timer reset.
