I found this achievement to be a bit of a pain.

I had previously edited the rosters of one team(Atlanta Braves) so the batters had the sliders all the way to the left for hitting, batter fundamentals, and fielding. This may be unnecessary, I couldn't tell if it had an affect on the achievement.

In the VIP settings, be sure the Throwing Interface is set to Total Control. Set the Pitching Interface to Precision.

I set the difficulty to Rookie. I also later discovered I had Throw Meter Difficulty (Under Difficulty > Human) set very low (about 5-10% of the bar). Based on the description, you may want this all the way to the left (0%).

I started up a 9 Inning exhibition game as the Boston Red Sox, playing at home against the Atlanta Braves.

You will start out as the pitching team. You will want to always walk the bases full with intentional walks (Hold RT and press down on the right thumbstick [RS]). This means there will always be three potential runners to throw out.

I found the most annoying part to be stopping myself from recording outs. Whichever fielders you aren't controlling will try to make the correct play on the ball. I found that even if I took control of an AI controlled fielder, he would still record an out. As you play, you should get the hang of not recording outs. This isn't that important, but it saves time when you can keep the AI batting. If you record three outs, you either have to restart the game or wait for the AI to record three outs (you can just make poor swings and strike out).

Whenever you have the bases loaded, throw fastballs over the heart of the plate. You can do this by holding the Fastball pitch button (usually A) until it starts vibrating a little bit, and releasing when the circle closes in on the pitch mark. If the pitch mark flashes, the pitch should be accurate and over the plate. The AI has an easier time hitting balls over the heart of the plate.

Keep throwing the fastballs with the bases loaded until you get a ball driven to the outfield. The timing is a little tricky. You never want to catch a ball for an out. Make sure it hits the ground. You can try waiting to pick the ball up until you get the perfect situation for throwing home, but it is probably best to just pick the ball up right away and throw it and hope for the best. To execute the Right Stick throw, you will want to hold the RS down to throw home, and release when it is about two-thirds full.

I only had one throw that was close before I finally unlocked this, so this can seem frustrating. The throw I got it on was actually a high pop up that I let drop in shallow center field. I quickly picked it up and gunned it home and got lucky.

I spent about 40 minutes trying to get that throw (I'd say I made about 25 attempts throwing home). I also spent a lot of time editing the rosters, but at least the rosters could potentially be used in other achievements.

I don't recommend this game at this time if you like a high completion percentage or completed games.
