There is one combat level in each world, and their names, colors and locations are as follows:

Forest: Wild Den - Gold door, climb pretty much straight up and a bit to the right from the entrance, all the way to the top.

City: Alleyway - Gold door, climb to the top of the level, then drop down on the right side. Across a gap from the Clocktower level.

Mansion: Store Room - Silver door, climb up the right side of the mansion and this will be the first door on the left, after you pass one on the right.

Laboratory: Containment - Silver door, in the gap to the left of the center room.

You may need to use the purple character to reach some of these as she has a triple jump which is needed to access some areas. For the actual levels, use the green character as he deals the most damage with each attack. The levels are quite chaotic and hard to write strategies for, you'll have to figure out what works best for you. Just avoid using the area attack (X + Y) and make sure to combo everything. Remember that killing an enemy with a Y attack will often spread dust on nearby walls, so don't forget to pick that up. Also do not kill the last enemy with a Y attack, as this instanly ends the level before you can pick up the dropped dust. It's usually easiest to leave one of the harmless floating ball enemies alive until the end for this purpose, although that's not really an option on Store Room, where you're better off saving the knight for last.

You can get this pretty early in the game providing you have at least two silver keys and two gold keys. If you're struggling getting SS on the early levels, just keep playing between the 16 open levels (four in each hub). Providing you get the SS for Containment (Laboratory) and Store Room (Mansion), you'll then be able to use both gold keys respectively to open up Wild Den (Forest) and Alleyway (City), which RazorPriest's solution gives the locations for.

Now, I should mention that using the area attack at any point during the combat stages, regardless of what your rank is at the end will void this achievement for your current playthrough, meaning you'll need to start a new save to go for this again. If you do accidently use it, back out to the dashboard so your progress won't save.

In terms of difficulty for the SS on each combat stage, I personally would rate them as such:
Store Room (books are the real problem here, but otherwise quite a calm stage)
Wild Den (hedgehog spikes may throw off your combo, and the head statues might jump into
Containment (toxic can might get you unexpectedly if you're not looking)
Alleyway (garbage bags and sneezing monsters are the worst!)

I should note that I'm far from being good at this game, I'd even consider getting a lucky break and finding strategies that worked for me. As long as you don't rush and keep a trash spot open just incase you miss a jump or an enemy, you'll find that this isn't too daunting.

Just keep practicing the route that works best for you, but also making changes along the way if you hit a wall. The video below is for routes I took, and also has explanations within about what to do, and what to focus on. It may not work for everyone, but it should give you an idea of how to go around each segment.
