As opposed to the console version where one has to beat the game at least once before unlocking Insane difficulty, it's possible to actually play at Insane difficulty on PC without ever having beaten the game before. This is accomplished using console commands. Beyond this, you can actually switch the difficulty mid-game without losing achievement progress, which helps to make the game much, much easier.

If you're playing through the campaign and don't want to deal with a particularly hard or frustrating spot—like the ambush from the lambent Wretches—just use the console to change the difficulty. Once you make it through that part, you can change it back before finishing the chapter and still have it count for the achievement! You could even play through an entire chapter on easy and switch the difficulty right before the end. As long as you end the chapter with the correct difficulty set, you'll get credit toward your achievement. This even works on co-op, though only the host needs to change the difficulty via the console.

Some console commands (typically labeled as cheats) will disable achievements. I do not recommend using those if you want to earn achievements. The commands I will list do not affect achievements.

Difficulty commands (use ` to access the console):

Casual: SetDifficultyLevelEasy
Hardcore: SetDifficultyLevelNormal
Insane: SetDifficultyLevelHard

I tested it out first on General RAAM. I started the chapter on easy and switched the difficulty to Hardcore. The achievement for defeating RAAM on Hardcore popped right after I killed him.

I also eventually tested it on the entire campaign. Except for times when my co-op partner forgot to switch the difficulty back via the console, everything worked perfectly. If you do happen to forget to switch the difficulty back before finishing a chapter, you can always look at the chapter selection screen. It will display icons listing the greatest difficulty beaten for each particular chapter.

It appears possible that changing the difficulty restores health of NPCs and player characters. I haven't been able to test this thoroughly, but what limited testing I have done has lead me to believe that this is the case. Because of this, changing the difficulty mid-fight will not help with achievements such as
