To gain this achievement (and all other "Big In..." achievements) each venue in the city must have a tick next to its name. This shows that the venue has been completed.
The venues are what you see after you select a city. The sets are what show up after you select a venue. For a venue to be complete you do NOT have to get 5 stars on all setlists, you just need to be able to finish all of the setlists successfully.
When all the venues in a city have a tick, the achievement will unlock.
A note for Moscow: this city is where you will find the Endless Setlist.
That means you will need to be able to finish all 58 "standard" songs, in one sitting, before you will be able to get the "Big In Moscow" achievement (roughly 4 and 1/2 hours work).
