You can get this in any campaign mode game, just get obliterated by the other team. I got this in one of the Division 1 games with a final score of something like 9-38. Just reset the urge to quit out when you start taking a beating.

If you want to deliberately lose, just let your QB get sacked, or throw bullet passes (tap the button instead of hold it) as soon as you hike the ball. Most of the time, it'll get intercepted or knocked away.
