EDITED 11/16/14: I've added more details about certain spots of the game and have also added a BOSS section.

For those wondering what the basic differences are between the lower (Casual, Survival) difficulties and this one:

1) Enemies are tougher (That is, they can take more damage.)
2) Enemies are more alert (thus making hiding more difficult).
3) Enemies are more numerous (especially noticeable during certain extended combat sequences in Chapters 6, 10, 11 and 13).
3b) Enemies are more aggressive (will almost always run after you and swipe multiple times). This speed increase pertains to standard, boss, and mini-boss characters.

4) Harder enemies appear sooner. (Ruvik clones are encountered for the first time in Chapter 3 and several more times afterwards as opposed to their limited appearances in later chapters on Casual and Survival. Also, enemies with firearms and metal masks appear sooner. Invisible enemies roam the subway station. Kamikaze enemies appear more.)

4b) The Ruvik "chase sequences" that occur in Chapter 9 last longer and will often be worsened by the addition of other armed enemies. Expect Ruvik to pursue you at least 3 times.

5) Increased damage taken (Two uninterrupted attacks from an enemy will kill you unless your health is upgraded the whole way). Traps will also damage you more.

6) Traps are more frequent (often occurring in places they do not when playing on lower difficulties). Take special care during Chapter 11 (construction site where you are ambushed).

6b) Mine traps are more difficult to disarm (blue bar is noticeably smaller).

7) Supplies are more scarce. This trait applies even to objects such as the hanging corpses used to bait the Shigyos (large underwater creatures) dealt with in Chapter 11.

Obviously all bosses on Nightmare mode are faster, stronger, and more intelligent.

1) THE SADIST [Chainsaw-wielding enemy]
Nothing too special here besides a big increase in his HP. As on other difficulties, he becomes quicker as he takes more damage. Stay on your toes. For Chapter 15, a Magnum with level 4 damage will drop him in 5-6 shots.

2) LAURA [Spider-woman / demon]
She is easily the hardest boss in the game, especially on Nightmare. I'd wager her speed is increased by 100-150%. The chapter 5 fight no longer includes explosive barrels so rely on your matches, torches, and the fire pits. The chapter 10 fight is no joke either. Watch out for well-hidden bear traps and Laura's faster teleporting. DO NOT ENGAGE HER WITH REGULAR WEAPONRY on a regular run. Save that for New Game+ if you want the extra gel and goodies.

3) NEUN AND ZEHN (giant brutes in Chapter 6)
They're idiots. Use that to your advantage. Beyond where the first brute emerges, there is a fireplace-looking structure where you can duck and hide. They cannot reach you here. Let Joseph pick away at them with his rifle and throw a grenade every once in a while. Easy.

4) SENTINEL (large dog in Chapter 6)
The Sentinel boss shows increased intelligence and special defense against Agony bolts. In my encounter, he was able to shake off 2 Explosive bolts and fight off the effects of a Shock bolt before either of them inflicted any real damage. I'm not sure about other bolt types, as I resorted to other methods (sniper rifle). His charges are much faster too. Keep moving. Do not attempt to defeat entirely.

4) THE KEEPER [Safe-Head]
Not too hard. He's faster but his attacks are easily avoidable. In chapter 7, you need only fight him 2 times to advance. Use Freeze or Shock bolts to delay him while you're cranking the final valve. In chapter 13 (freezer battle), you'll have to defeat him 3 times to advance. Hang right near the entrance to the room. Agony Bolts and Magnum are your best friends here. For the final fight in chapter 15 (two Keepers at once), try Explosive Bolts and your level 4 Magnum. If you can trap them on either side of the center gate, pick away with your Sniper Rifle as well.

5) TRAUMA (mutants with barbed wire attachments)
Very dangerous. Use something to delay them (freeze or shock bolts) and then blow the hell out of them with anything explosive you might have. *Without spoiling where, BE AWARE THAT YOU WILL FACE THEM MORE OFTEN ON NIGHTMARE MODE.*

6) AMALGAM ALPHA [Mutated beast in parking garage]
This fight is not as awful as you might expect. Focus on shooting the creature's eye ONE TIME and then hiding. Rinse and repeat. If it detects you, do not panic. Sprint in wide circles when it charges, and then pick away at it with your Sniper Rifle and Explosive Bolts.

7) HERESY [Giant spider creature]
He's a little tougher and will use more varied attacks (overhead swipes and a deadly drag move), but other than that he is just the same as always. Sniper Rifle and Shotgun are useful here. Use grenades or Explosive Bolts to defeat the slugs he spawns.

8) QUELL [Tentacled creature in sewers]
Increased health is the only difference I noticed. At this late point, you should have some decent upgrades to your Agony bolts and Magnum. Weaken him with these and then shotgun the bastard until he's dead.

You will have to defeat his "claws" 5 times with the mounted machine gun (scripted sequence). For the rocket launcher finale, you can afford maybe 1-2 misses or you're toast. The ending QTE also happens faster so fire that one bullet quickly or it's lunchtime for Ruvik.

1) If you're going to be attempting Akumu mode, then Nightmare is wonderful practice for it. With that in mind, I would not worry about upgrading health too much. Your precious Green Gel can be spent wisely on other abilities.

2) Use the environment to your advantage. This includes shooting mines to take out multiple enemies or luring enemies through explosive/bear traps. Sure-fire way to save Seb some ammo in earlier chapters.

3) MOVE SLOWLY. TAKE YOUR TIME. If you're trying to do a speed run, you will only end up frustrating yourself.


***I do not recommend doing Nightmare difficulty for the "It Is What It Is" achievement. However, I'm certain it is possible, as Seraphim17 has accomplished this feat on AKUMU mode.***

1) AMMO STOCK (especially Agony Bolts and Shotgun shells and maybe Pistol ammo)
2) MATCHES (be able to carry AT LEAST 15)
3) SPRINT (Rec. level 3-4 by Chapter 6)
4) AGONY BOLTS (Flash and Shock bolts are your life-savers. Level 5 Harpoon bolts also come in handy for later chapters but the level 5 upgrade is pretty expensive at 50000).
5) SNIPER RIFLE (Rec. level 3-4 damage, 2-3 reload, 2-3 firing rate. This weapon will make bosses much more manageable, especially the Alpha Amalgam in Chapter 10).
6) SHOTGUN (Rec. level 2-3 damage and 2-3 clip size. Best used as a knock-down-and-burn-them weapon.)
7) MAGNUM (Rec. level 4-5 damage if you can afford it. This weapon's late appearance can make it your go-to method for Chapter 15's Sadist and Keeper fights.)
