A word of warning:
You cannot use the infinate launcher or samurai edge (both unlocked by completing the game on normal in less than 3 hours) in real survival mode (also unlocked by completing the story on normal) or Invisible enemy mode (unlocked by beating real survivor mode). The weapons are in your inventory and the item box in hard mode or lower but not in real survival mode. You can however get the hard achievement by doing real survival mode.

The reason I created this post is that I was under the impression that samurai edge and launcher would be available in both modes once unlocked.

Sorry this is not a proper guide but I wanted to give people a heads up as to what they will be going for when attempting this achievement.

This is the order of all seven of the playthroughs I did and what I unlocked each time.

1st - chris very easy -
just to get a feel for the game.
this got me all of chris' story related achievements.
I also got
'Break Out the Marshmallows!' setting fire to 2 zombies,
'Delaying the Inevitable' give Richard his serum and
'Every Nook and Cranny' - visit all places on the map (this means go in every room/area and pick up every item so the room is no longer the color red on the map).
By saving Rebecca and jill I also got
'The Nightmare Ends'.
By reloading a save before I unlocked jill's cell and not saving Rebecca on the helipad at the end I also got
'Every Man for Himself' finish the game without saving anyone.
This playthrough unlocks the 1st of chris alternate costumes also.

2nd - jill very easy -
This got me all of jill's story related achievements.
By giving barry his gun back and unlocking chris from his cell you get
'I'm a Member of S.T.A.R.S.' which is save both chris & barry and complete the game.
By reloading a past save at the point where you give barry his gun back say no and then don't unlock chris from his cell and complete the game gets you
'Every Woman for Herself'.
This playthrough unlocks jill's 1st alternate costume.

3rd - jill normal -
Now I had a feel for the game I completed it in under three hours for
'Take that, Zombies!' & 'Not in the Mood to Die' (completing game on easy & normal) and
'Don't Stop Running' & 'Racing and Pacing' (complete in under 3 hours & five hours).
By doing the speed run on it normal unlocks you the samurai edge and rocket launcher.
As long as you load all three grenade launcher types and barry's magnum in jill's run it got me
'Starsenal' obtain all weapons.
This also unlocks jill's 2nd and final alternate costume.

4th - chris very easy -
With the launcher infinite ammo this was a breeze. I got the
'ink is for squids' by not saving for the entire play-through and
'Passion for Fashion' by getting chris' 2nd and final costume and getting you all the alternate costumes in the game.

5th - jill very easy -
By playing through and only using the knife and no defensive weapons or any other weapon bags you
This is a lot easier than you think, especially as by now you know the game quite well.
I killed key zombies in the mansion like the ones by the save rooms as I knew they would be an issue. hunters were not so hard to kill with the knife as I thought they would be. the crim head proto was hard and I used all 6 health items I had to beat him. yawn 2 was easier but only by a little and tyrant was the easiest I found taking only 20 or so up slashes to beat him and only making me use 2 health items in the process. in the end I had about 8 full health items remaining. didn't pick up 1 piece of ammo and of course I had defense items on manual so I didn't risk using them although I didn't pick up any anyway. I didn't give barry his gun and left chris behind so I didn't face the tyrant again on the helipad.

6th - jill real surviver mode -
'The Survival Horror' & 'Oh, the Horror'
By doing this you also get complete the game on hard.
This is the big one, the one all the other play-throughs were leading up to. You should be ready for this by now. I did it 4.5 hours. the tyrant went down with just 1 shot from Barry's magnum. The hardest part was making trips just to move items from one box to the next as in real survival mode item box's are area specific, meaning they don't warp to other boxes when you put things in and auto-aim is disabled.
Damage dealt and received in hard and real survivor is actually the same as normal mode. The major and apparently only difference to normal mode is that you get less health and ammo pick-ups in real survivor.
Getting this achievement was no where near as hard as I thought it would be. By saving this until after you have done the speed-run on normal you will know the enemy placements and so on.
There is limited ammo and health and ink ribbons but just save often when you know you have done well or you know I boss fight is coming.
It didn't feel much harder than normal to be honest, you just have to be more careful with health and ammo.
Doing this mode unlocks the final 'Invisible Enemy' mode.

7th - jill very easy - invisible enemy mode.
Complete the game on invisible enemy mode and get
'Ghost of a Chance'.
This is basically the victory lap as with a large amount of ammo, health and defensive weapons as well as your experience in all the other modes and methods of play this should be very easy for you to do.
Like Real survivor mode you CANNOT use the infinate launcher or samurai edge if you unlocked it by beating the game on normal in under 3 hours. But that is not really an issue as you can play through on very easy anyway and have lots of health and ammo all the time. Also the auto-aim is disabled for this mode as well.

I realize that potentially you do not need to do that many play-throughs but I found it easier to break it up this way. Either way you will need to do an under 3 hour normal speed run for the weapons and play each character twice for the costumes, so you will be looking at a minimum of 4 play-throughs anyway. Once you start mastering the game you can do a playthrough in 3 or so hours anyway so that's still 12 hours minimum if you play 4 times. I did 7 so that's about 21 hours for me give or take for the 100% so that's not to bad in my opinion.
