The easiest way for me, was this:

    After completing:
Grand Theft Auto: San AndreasSecret AchievementThe Secret Achievement achievement in Grand Theft Auto: San Andreas worth 65 pointsContinue playing to unlock this secret achievement

You will unlock "San Fierro" and also you will respawn here, in Angel Pine:

And in this exactly point, is the ambulance:

Is very very easy at this point, because you only have to be around in that small city, and believe me you will save around 30 minutes or even more.

    Mission Failure
    - The Ambulance Falls Into Water.
    - The Ambulance Blows Up.
    - The Ambulance Flips Onto It's Roof.
    - You Kill A Passenger. (Running them over or crashing the Ambulance too much)
    - You Are Arrested.
    - You Exit The Vehicle.
    - You Push cn_RS.
    - Your Time Runs Out.

    Completion
You will need to complete Level 12 of the Paramedic Missions in order to finish this Sub-Mission. Level 12 must be done consecutively, and altogether you will save 78 people, however this may increase in your stats if you fail or cancel the mission. The average time for completion of the Paramedic Missions ranges from about 30 minutes to 1 hour and 30 minutes, depending on location, however I'd advise Angel Pine if you can get there.

    Rewards
-Upon completion of Level 12 your health bar will be increased to it's maximum, resulting in your health raising by double or more. This of course helps greatly on missions, or random gunfights with enemy gangs or the cops.
    -And of course the achievement.

    Fluke939Couple of tips

Before you start, punch the rear and and right side doors of the ambulance until the doors come off, saves a second or two each time a patient gets in or out the ambulance.

When dropping off, brake with the handbrake (X button) don't do what I did, breaking with LT, patient gets out the back, I'm still holding brake, it starts reversing, patient is run over and killed before registering as rescued.

JahonCrossRS is an annoyance, having it be held for a moment would have been a better option.

YzahkinI died near this place, woke up at that hospital and I remembered this guide, so I took the opportunity. :D Also good spawn point for the Sanchez bike for the A Legitimate Business achievement. So I just took the bike too. :D

Want to get this early after the first mission? No problem! Near your Grove Street safehouse, there's a hospital right nearby with a parked ambulance.

You'll want to take it all the way up to Fern Ridge located below:

Start the mission by clicking in cn_RS, and pick up the nearby passengers, making sure to slow your movement upon reaching them. Make sure there's enough space behind you for them to get into the back.

Paramedic is more about keeping cool under pressure than being a full-on drag race. You get plenty of time in the much later rounds making return trips less of a problem.

There are 12 rounds in total which must be completed in one sitting.
The average time to complete it in this area will take 35 minutes.
You cannot leave the ambulance or kill a passenger as these are fail states.

Completing them all will give you a permanent health boost, which is really useful this early, and more so later on! Here are some points you should keep in mind when you're making your way around.

    * AI drivers will always come to a halt when you're picking up passengers, however you should be careful about your surroundings as it is possible for the passenger pathing to get caught in a mess if there's no way for them to get to your ambulance. Case in point, having a vehicle block your back doors.

    * The ambulance is prone to tipping over if you spin out. Going over small hills and bumps is never a viable option even if you want to try taking detours/shortcuts. You should always follow the road and mind incoming cars to avoid potentially killing your passengers and damaging your ambulance. It can only take so much punishment until it catches fire.

    * Try to keep your thumb away from the cn_RS to control the camera. Yes, it's a state of habit, but it can also potentially ruin the whole mission if you click it in. Take your corners carefully and let the camera swivel back to your rear.

    * Plan your routes in advance. Check the map at the start of each round, look at where your passengers are spread out and keep your waypoint at turns. You don't always need to pick up 3 passengers. If there are 2 in one town, and four in another, get them in 3 groups of two. You'll learn pretty fast how crucial it is to get them back in even numbers.

    * When driving up hills, it may be difficult to see vehicles in front of you. Always take it slow in this case as once again, time isn't an issue, and you'll more than likely collide with someone.

    * Rain and stormy weather will impact your driving. This is something that occurs randomly during the in-game time, but this will make your trips slightly more tedious.

    * Watch out for cops. Always.
