To obtain a level 4 weapon first you must get your Smithy up to level 4. To do this, in the strategy phase, select Decree > Armaments, and then select either Swordsmith, Spearsmith or Specialsmith, depending on what kind of weapon your character is using. Keep doing this until you reach level 4.

Now that you are level 4, you have to get at least 500 K.O.s in one battle. To do this start a battle phase and set your army to defend by pressing Down on the D-Pad. Now go straight to the enemies main camp, don't take any bases or anything else on the way. Because you haven't secured any other bases this stops the main enemy ruler from appearing and also means that the enemy camp can't be overtaken so it continuously spawns enemies. Keep killing enemies here until you reach at least 500 K.O.s, note that you won't receive any sort of notification that you have got the weapon.

Now finish the stage successfully and once the stage is complete you will obtain the weapon in your spoils. Don't forget to save to unlock the achievement!

If you're having trouble finishing the stage in time, or beating all the enemies/dealing with all the enemy officers going to your location, I would recommend either waiting to do this until you are a higher level with more developed weapons and using the Extend Time tactic. Extend Time gives you 10 extra minutes of play, this can be bought during the Strategy phase by selecting Tactics > Advantage, and then applied in the pre-battle menu. Failing that you could also set your games difficulty to Novice (the easiest difficulty), but I'm pretty sure you can only do that at the very start of the game, not mid-play.
