There are currently 9 Combat Ranks:

Harmless, Mostly Harmless, Novice, Competent, Expert, Master, Dangerous, Deadly, Elite.

To progress through each rank you need to defeat enemy ships. It doesn't matter which ships you defeat, as long as you deal the final blow. For every enemy you defeat that is the same combat rank as you, you gain 1% towards your next Combat Rank. The lower your Combat Rank and the higher your opponent's Combat Rank progresses you faster e.g. if you are Harmless and your foe is Elite then you get 3% progression towards the next Combat Rank. And vice versa, the higher your combat rank is to your enemies, the less you progress e.g. an Expert destroying a Harmless vessel won't gain any progression.

Theoretically if you only ever fight ships at your Combat Rank you can do this in 800 kills, that's one hell of a grind. Most of these will come naturally but towards the rank of Master you will find it difficult to either (A) find enemy NPCs on solo that are of a high enough Combat Rank or (B) find another player who isn't insanely good. I prefer solo play personally so I find good places to find targets are Nav Beacons and Resource Extraction Sites, once there find ships that you can take on and rack up the progress. Just remember to see if the fight is really worth it.

Update as of May 2016
Since the latest update 1.6 NPC enemies are much tougher to beat and even some Mostly Harmless pilots are giving novices a run for their money. I have heard of plans to nerf them so they're not so deadly but until then I'd suggest you stay away from high combat ranked ships unless you're 100% confident in taking them down.
