As implied in the description, this should unlock when you are the first to discover (meaning find it as Unknown), scan it (uncover its planetary name) and then sell the data (move 20+ LY away from the location and use the Universal Cartographics feature at any station with it.) Of note, make sure its a planet and not a star, moon or gas giant. Only an actual planet will trigger this achievement.

How do you actually go about finding an undiscovered planet? Initially, it would be incredibly hit or miss since you need some serious dough to buy an Advanced Discovery scanner. There is much debate over what ship to use - general consensus is the Asp but others suggest going with the Cobra Mk III. Having messed around with the game more, I'm now jumping from star to star in a Diamondback Scout with the Detailed Surface Scanner and the Advanced Discovery Scanner. Using both will not only scan the whole system but also increase the payout for the objects that you do specific scans on.

For reference:
Detailed Surface Scanner: 250,000cr
Intermediate Discovery Scanner: 505,000cr
Advanced Discovery Scanner: 1,545,000cr

Where do I find parts?
The best place to look for parts is a system with a High Tech economy.

Update on parts - you can't actually move them at this time from what I see but you can transfer them at a station using the following method assuming both ships are parked at that location.
1) Sell equipment you want to transfer.
2) Store the ship you sold parts from and retrieve the ship you wish to upgrade.
3) Go to Outfitting and when you select the hardpoint you wish to upgrade you will see 'rebuy' as an option on top. GRAB ANYTHING YOU WANT TO SAVE NOW! Once you leave the station it will be full price again.

Once you have your Discovery Scanner, equip it to a Fire Group since you 'fire' it with cn_RT and go to a new system. Once there, fire the scanner and see what you find. Its worth noting that only the Advanced Discovery Scanner will ping a whole system. The Intermediate Scanner has a range of about 1,000LS. This means if you are a junior explorer you may have to travel around the system and scan several times to get the whole picture.

We have joined the background simulation that the PC and Mac players have been in for some time. Although the database is the same, we can not interact with each other. It could take more effort to find a planet to call your own since this galaxy just got a bit more crowded. Of note, I still commonly find unclaimed planets less than 1,000LY from the bubble with my closest find being less than 100LY from Orang.
