Just a quick solution for people that are stuck. This is how I unlocked it.

You can get both characters out by using a pickaxe (crowbar + duct tape + tool handle, which can be made by combining timber and a file) on the wall of the room they are locked into.
I recommend upgrading your pickaxe when it only has one use left by combining pickaxe, timber and duct tape so you don't have to find another crowbar.

You might need the blue key to get to Michonne but I think you can get there by walking outside of the fences (just inside the outer wall) (confirmation needed)
You might also need a purple key to get to Glenn's wall. (confirmation needed)

The purple key is located in the top right building (The one where the Governor is) in the chest behind the purple door, you can get here by breaking trough the wall with a pickaxe or going trough the vents in this building.

The location of the blue key is still needed for this guide.

After breaking out Michonne you need to take her to the Governor before escaping Woodbury

Escaping Woodbury can be done by using pliers on the fences of the outer wall (There are fences on the east and west side).
I recommend taking 1 or 2 working radio's (depleted radio + battery) with you to distract zombie's. And a modified weapon just to be sure.

I also didn't really worry about dying since you keep all your items and the zombies never really pose a real danger since I only saw them around the outer wall and in a couple of buildings. Even when my threat level was 100% the only time I really needed to get past walkers was when cutting trough the fence or when a guard I killed turned.

Using a working radio to distract the zombies will also net you this achievement if you haven't already unlocked it.
