I tested and found a fairly simple way to obtain this achievement. There is some random luck involved here but this method should work.

While it is true you have to evolve Total Brian Death in the first year, what the achievement doesn't say that helps is you can de-evolve it after and STILL get this achievement.

I found the easiest way to beat this scenario is play Simian Flu on Casual. Here is the setup I used if you are curious.

ATP Boost
Ion Surge
Together Strong

I started in Saudi Arabia for access to air and water transmission

Start right away working towards Simian Neuro-Genesis. From there buy the following items de-evolving everything but Neuro-Enhancement 1 and 2 if they happen to evolve...

Ape Colonies
Social Cohesion
Cognitive Shift
Ape Rampage
Organized Travel

Place a colony in Saudi Arabia right away once you have Education and if your plague has spread and you have intelligent apes in another country get one there as well. This is to help you build more DNA points

Now your focus in spreading the plague as fast as you can. Evolve the following while making sure your timeline doesn't pass 11 months from your start date to be safe and keep de-evolving all mutations but Neuro-Enhancement 1 and 2

Air 1
Water 1
Drug Resistance 1
Cold 1
Air 2
Water 2

By this point you should be close to a month before a year from the start date. If you are low on DNA let it pile up now so you can prepare for the end of the month. If you have excess, evolve a few travel upgrades or Air 2/Water 2. Bird 1 and Insect 1 don't hurt either. Keep in mind you need around 35 to 40 DNA to evolve all the way to Total Brian Death. Once you hit about 10 days before a year from the start date, evolve Neuro-Enhancement 1, 2, 3, AND Total Brian Death. Let it run for about 3-4 days in game time and then DE-EVOLVE Total Brain Death and Neuro-Enhancement 3.

If you did this quick enough, not too many people died due to the upgrade and the cure should only just be starting. Keep evolving anything that transmits the disease (Air 2, Water 2, Bird 1, Insect 1, Drug Resistance 2, Cold 2). Keep adding another 2 ape colonies to help speed up your DNA collection. Start now upgrading your Ape Rampage attacks (Primal Hunt & Weapon Creation) and all your movement abilities (Logistics, Seafaring, & Horseback Riding).

Keep an eye on labs that are working on the cure and move your colonies to those countries and Rampage them to destroy the labs and slow down the cure. Feel free to put some DNA points into Genetic Hardening 1 & 2 to help slow down the cure also. Avoid drones and wait until you get the pop that says there is no healthy people left in the world. Now just upgrade Total Brain Death again and any other lethal symptom you can get your hands on while hunting down Labs to slow the cure. Once the population dies off, pop goes the achievement.
