The very beginning opening sequence of the game will have you waking up in a room with some other people, the game will then tell you to blink to clear your vision (Right Stick) DO NOT click the stick. Your vision will stay blurry and the scene will continue as normal, they will have you walking down several hallways and flights of stairs, eventually you will make your way to a small room with a mirror which the game will then takeover and the achievement will pop.
