its just that simple just play 100 online ranked matches - make sure that you complete the game you are in and it doesn't matter if you join in the middle

To add to Lost Raven's guide, while you are in the game you must be active in it meaning you cannot simply join a game and then go do something else and leave the xbox running. I don't know exactly how it works but if you just leave the xbox running while in a multiplayer game and you are not actually moving or playing the game you get kicked from the multiplayer game
