Best done in Instant Action with bots on Novice difficulty.
Here is a list of all the Mutators. Most of the Mutators are availiable in all game types, but there are some that are only availiable in one or two game types, so if you keep playing over and over trying to get this achievement and never get it, that's why (and don't feel bad it took me a while to figure that out too). For all such mutators in my list I will tell you which gametype it is availiable in. Here goes:
Big Head (all types)
Friendly Fire (all types)
Handicap (all types)
Kills Slow Time (all types)
No Super Pickups (all types)
Slo Mo (all types)
Speed Freak (all types)
Super Berserk (all types)
Weapon Replacement (all types)
No Orbs (Warfare ONLY)
Survival (Duel ONLY)
Low Gravity (every type EXCEPT Vehicle CTF and Warfare)
Weapons Respawn (CTF, Deathmatch, Team Deathmatch ONLY)
Instagib (every type EXCPET Vehicle CTF and Warfare)
No Translocator (Capture the Flag ONLY)
