*** This is an incredibly poor piece of game design from D3. A quick Google search on "Where do I go after Araveine?" gives you an idea of how many gamers have been left completely in the dark by this part of the DLC as there are no 'red' main storyline quests in sight!

Given the amount of gamers (like myself) that had trouble with this, I'm posting this up as a seperate solution, rather than just a comment against Boiled Frogs (although everything he has said is correct, there's a chunk of info missing.) ***

To unlock this part of the storyline you will need to have defeated Araveine and completed all of the sidequests in the K'Tarn, Lysea and Stormreach areas. In addition to this there is a side quest mission at Gallia where you have to go and kill (or lie about killing) a medusa. Once you have done all of these, travel all the way round to Gluk and see Drong.

Drong has a bunch of food side quests which you may or may not have completed during the main game (I hadn't). You need to get all of these out of the way and then, finally, a mission called Stinky Valley will unlock. Do this mission for Drong and then return to Gluk where you will finally see a red exclamation mark and can continue through with the main story quests.

When you see Antharg, give him a kicking from me.

I have a few more details on how to unlock this achievement. I'm doing this as a separate solution because its a little more in depth. Boiled Frogs and BusyRich are on the right track though.

You need to have defeated Araveine.
You need to have finished side quests for K'Tarn, Lysea and Stormreach.
You need to kill the Medusa for Gallia.
Proceed to Gluk and do food side quests for Drong.
He will then talk about the stinky valley. Clear out the Plague Tasters and also Plague Priests along the way to Khaz-Barak.
There is a Plague Priest there as well. defeat him and make your way to Gallia to continue the missions to raid gold from Khaz-Barak.
Shortly after that you will need to travel to K'Tarn and obtain the quest "Against The Plague." You will need to be LEVEL 45 to take on this quest!
Travel to the Rotting Shrine.
You will fight a Plague Witch. After you defeat her you will open a path to the Citadel of Plague.
There you will challenge and battle Lord Antharg.
After you win you will receive the Tome of Plague and +1000xp.

Another storyline based achievement. You may have some trouble carrying on with the main storyline, well I did. However after travelling to Gluk you can continue the main quest, which should inevitably lead to Antharg.

He's no big deal, the only thing that's really annoying is his armour which halves all the damage you do to him, just keep piling it on. Personally I found Bane harder

The key to beating Antharg is to prevent him from casting his spells, all of which drain your mana. Without my mana, I was powerless to do any damage to him beyond whatever skulls I could match, and that wasn't enough to beat him. All of his spells require yellow mana, so match all the yellow gems that you can to prevent him from getting them.

It's also handy to have a low-level battle spell, like Fire Bolt, that you can still use in case he does drain your mana to very low levels. Spells that cause him to lose a turn are useful, as well.
