You will get this once you have obtained all 'Medals' under the National section of the Trophy Case. There are 50(!!!), these are basically split into the continents (minus Antartica making 5).
You need to get Assist Leader, Top Scorer, Team of the Tournament, Player of the Tournament and the Cup winners trophy for these:
European Cup
African Cup
South American Cup
North American Cup
Asia-Oceanian Cup
Thats 25 medals there. You also need to get Top Scorer, Assist Leader, Team of the Qualifying Round and Player of the Qualifying Round. You can earn 1 set of these per Legend you have as you will have to qualify for the International Cup from each region/continent.
So thats another 20, the remaining 5 are for getting...guess what....Top Scorer, Assist Leader, Team of the Tournament, Player of the Tournament and the Winners Cup for the International Cup (aka World Cup).
I recommend following Iced Hawk's advice about simming games in his solution for All Titles. This will take a while but you will get it on your way to the All Titles and Ultimate Player achievements.
