You will get this once you have obtained all 'Medals' under the Club section of the Trophy Case. There are 10, these are:

League A / B / C / D - Winner
League A / B / C / D - D1 Cup Winner
European Championship Winner
European Masters Cup Winner

This can be achieved over the course of different legends. I won all League A and D titles with my Japanese dude and all others with my Mexican man on a different gamesave. You should pick this up on your way to the All Titles achievement.
