As far as I can tell, you can't fail a song at sophomore difficulty (I bottomed out at D-), so choose one of the higher classes. Don't touch the controller at all and at the end of the song you should get an "I" grade for incomplete and the achievement will pop.

I can't imagine this achievement really counts as a mark of shame. This game is so easy that you actually have to make an effort to fail a song.
