Go through Act 4, Chapter 6. Battle through the various enemies and such. You will have to fight through a few floors of Locusts and Lamberts, when you reach the objective of gaining entry to the Throne Room, do so and the achievement will pop up during the cutscene as you enter the Throne Room and the Queen. This achievement will come along the way as you progress through the game.
