Get 100 Critical Triggers to unlock this achievement. You should unlock it early (it should be your first) in the story.

When a Critical Trigger initiates, the battle will slow and either the X,Y,A,B (as a group) or the Right Trigger will appear on the screen and a circle will appear and start to close on the target button. Hit the target button right as the ring closes to get the Critical Trigger. Too early, too late, or hitting the wrong button will cause it to fail. You can get these both attacking and defending.

Once you succeed in one, your next attack will almost always be a critical trigger, which makes this even easier to unlock. There's also a good chance it will carry over from one union to another.

as Codster2005 said that can be done the "Manual" way. You can also enable the automatic trigger via options menu meaning the pressing of buttons will be done by the AI itself. This will unlock even before you step on disc 2 on normal gameplay.
