FURTHER UPDATE: Despite this achievement now being obtainable, it is known to be buggy and not unlock even when the requirements have been met. CRISTIAMUZ got his achievement to unlock by playing a non-ranked spot battle. So if it won't unlock even after you've met the requirements, try playing an unranked match.

UPDATE: August 2017: No-one has won this achievement, or any of the other rank achievements in a very long time, so it may actually be the case that this game has a number of discontinued achievements. I have not confirmed this myself, but there is mounting evidence pointing to this being the case. Be aware of this if you are considering starting the game. All that said, as I understand it some people did unlock this after the rank system went offline, and as far as I know nothing has changed since then, so this may well still be obtainable.

ORIGINAL SOLUTION:
I am going to try and put together a 'modern' guide for this achievement, since the circumstances surrounding it have changed a lot since these other guides were written! It's likely that no-one will read this but I hope it's useful to someone! Click the spoiler tags to reveal massive amounts of information!
This achievement used to be time-consuming/difficult, but recently has become almost impossible! The reason for this is that EA have managed to mess up the rank/medal/leaderboard system on the game, so that now you cannot see your rank, what medals you have achieved, or even how much exp you have in each game mode! To put this explicitly, I'll draw up a little table, with an representing 'can check' and a representing 'cannot check'

So clearly we now have access to a lot less useful information about our progress! If an analogy for this achievement was walking across a room, now we have to do it in the dark!
But the actual requirements for the achievement are still exactly the same! That is to say: 5 Bronze Medals, 3 Silver Medals, 1 Gold Medal and 5000 total EXP.
To break this down, a Bronze Medal is awarded for 100 EXP in a gamemode, a Silver Medal is awarded for 500 EXP in a gamemode, and a Gold Medal is awarded for 1000 EXP in a gamemode.
The gamemodes are as follows: Best Trick, Spot Battle, Jam, Deathrace, Hall of Meat, and S.K.A.T.E.
So you will need to play 5 out of 6 of these gamemodes to get the required number of medals (most people choose to avoid deathrace)
"Ah, but pezza", I hear you say, "how on earth can we still get this achievement if we can't see the progress on medals or rank?", well the progress is still made in the background, we just cannot see it. So a new method is required, what I like to call the "Pathfinding Method":

0. Decide on what medals you will go for in each event. This means decide on one event to not play at all, two events to play to bronze level, two events to play to silver level, and one event to play to gold level and beyond
1. Check your total EXP via the leaderboards. This is the only statistic that is still visible, so we have to make the most of it
2. Write down your total EXP
3. Choose a gamemode. You should start with the gamemodes you have chosen to get bronze in and then work your way up through the medals in increasing order
4. Go away and play your chosen gamemode, it may be advisable, especially at this stage in the game's life, to boost with a friend
5. When you're done playing, check the leaderboards again, and write down your new total EXP
6. Find the difference between the initial and final EXP to obtain a value for EXP earned
7. Allocate this EXP earned to that gamemode by writing it down on your sheet
8. Repeat the above steps for your chosen gamemode until you have earned enough EXP that you have got all the medals you need for that gamemode. (e.g. keep playing JAM till you get, say, 500 EXP)
9. Move onto the next gamemode and repeat the above again, playing till you have the required level of EXP in that mode
10. Continue until you have, according to your sheet of paper, achieved taken 2 events to silver level (500+ EXP) and 2 different events to bronze level (100+ EXP)
11. Now the real fun begins. It's time to start on your gold medal event. Since at this point your other events should be up to their required level, you can actually just play your chosen gold medal event from now on. If you don't want to, you no longer need to track EXP at this point, because you are going to keep playing the same event until the achievement pops! You should have chosen your gold medal event to be the one that is quickest for you to earn EXP, because you have to gain the most EXP in that event! Most people pick 'Hall of Meat' as their gold medal event. If you like you can still keep track of the EXP won so you know when you have won gold.
12. If you tracked it all the way, then by the time you have all the medals you need, you should have at least 2200 EXP on the EXP leaderboard (working: 1x1000+2x500+2x100=2200). At one point this would have been known as 'Icon' rank, but as we have no way to check rank we cannot confirm whether we have got enough medals or not, hence the need for a paper system!
13. Now we just need 5000 EXP. Depending on how you are following the guide this diverges a bit:
(i) "I didn't track gold EXP, but I know I have got the other 2 bronzes and 2 silvers": You need to keep playing your 'gold event' until the achievement pops, once the leaderboard reaches 5000 EXP, the achievement will unlock
(ii) "I did track gold EXP, so know I know that I definitely have 2 bronzes, 2 silvers and 1 gold": You can now play any event you like. It doesn't matter at all. Ideally you should play your gold event anyway, since you chose the gold event to be the fastest for EXP (many recommend Hall of Meat)
I feel like I've rambled a bit there, so below is a TL;DR,
TL;DR: Play online game modes with reference to the leaderboards until you are certain you have met the medal conditions for each mode, that is to get two to 100 EXP, two others to 500 EXP, and one to 1000 EXP, then keep playing to 5000 EXP.
The specific method in terms of which gamemodes are required is up to you but below is an example of how you would do it:
