As straight forward as it sounds. Complete 10 missions in a row without failing. On recruit difficulty, it is extremley easy to achieve this through your general play through.

Note that if the mission is interupted (Dawn of War crash for example) will sometimes reset your missions in a row.

You can generally only fail a mission by having all your commanders unconcious, or in the case of defence missions, having all the generators destroyed.
