I did this on chapter 2, easy mode. You have no choice other than to play as Thomas. "Severely Wounded" means when the screen goes fuzzy with a red outline and sounds are distorted. If this starts to happen, hit the start menu and reload the checkpoint.
Use your rifle, keep your distance, and creep forward into cover slowly. In firefights, hide and let Ray do most of the fighting.
When you get to the area with the corn field, scout the area with your rifle crosshair first - it has a better range than your knives and will flash red when you're facing an enemy. Switch to your knives and make the kill.
Funnily enough, during the last part with the steam boat, I was one-shotted with a cannon and killed. The checkpoint reloaded naturally and I completed the level to still get the achievement. |

| If you hear heaving breathing and the heavy bass of your heartbeat rumbling through the speakers you missed it. Restart and try again. Best to do on Easy moving from cover to cover on one of the earlier levels. |
