This Achievement is simple enough, all you have to do is complete the game as Sonic. That means completing Mushroom Hill, Flying Battery, Sandopolis, Lava Reef, Hidden Palace, Sky Sanctuary, Death Egg and the final boss. Considering that you can save anywhere in the game from the pause menu, you shouldn't have a problem with lives - if you think you are going to die, save it and if you do, reload the game.
