I got this achievement pretty early on in the game. There is a task you have to undertake whereby you have to get a key out of 1 of 4 bodies. You have to look at the x-rays on the way to decide which body has the key in. Once you know which one has it you use a scalpel to cut them open.

Now, just keep a hold of the scalpel and you soon get to a bit where the ingame tutorial tells you how to bolt the door shut and block it with an object. If you don't do this an enemy will come through and start atacking you. Just kill him with the scalpel and you will get the achievement. I only had to hit him once with it for him to die
