Right at the beginning of the game (directly after "S.S.O.D") you will be on the "Team Player" mission. At the start of this you will be told to keep the pressure on the RPG teams across the river. Shortly after this you will be instructed to switch over to your MT203 Grenade Launcer (Press LEFT on the directional pad). No fire your grenades over the river and you should be able to pick up this achievement.
