Ok I'm going to add my own solution because there are a few details left out.

*************Spoiler Warning******************

In every instance there are 2 factions you can pick. You only need to have recruited one of the two to unlock this achievement.

You have the choice of the
Werewolves or the Dalish Elves
Dwarfs or the Golems
Circle of Magi or the Templars
Humans


The humans are the only one that as far as I know you don't have a choice about as I believe you have to save the Arl of Redcliffe. This achievment will unlock once you go to "landsmeet". Hope this helps.
