This can be done with at least two players by using two controllers or playing with a player online.
You do not need to do every level just the listed below but you must complete the game at least once to be able to skip chapters. (This also applies for the single player achievements, just do these levels)

Chapter 3
Chapter 4 - Conclusion
Chapter 5 - Conclusion
Chapter 6
Chapter 7 - Conclusion
Chapter 8
Chapter 9
Chapter 10 - Conclusion
Chapter 11
Chapter 12 - Conclusion
Chapter 13
Chapter 14 - Conclusion
Chapter 15 - Epilogue
Difficulties do not stack so you wont get the achievement for medium and easy if you do hard mode straight off the bat.
