The "Pale Hearts" quest is obtained from Sam at the shop of occult (near the cathedral in inner city).
Once you've gathered your 9 hearts go back to Sam, the achievement will unlock while talking to her.

From the game official forum :
Outer District: Garden in front of Hooded Wings
Inner District: Rooftops (The one you fight when you search the dead body)
Inner District: In the area that is reachable only from catocombs (the Hooded Wings Quest: Painting location)
Arsenal Distrcit: Enter the Sailors' rooms across the bridge from the Skullbreak Tavern, Use Twilight Portal and go out of the door.
Arsenal District: Go to the Street Vendor then continue past him to the blocked bridge. Near the bridge
Port Distrcit: African Ship side of the bridge
Port Distrcit: Warehouse side of the bridge
Eastern Villa: Go to the garden where you dig up the story relevant thing.
Palace: One of the ground floor chambers that you can access before proving your loylity has it.
