Simple! Host or join a CONTROL POINT MODE game. Either boost with a friend or try to find someone online. Once it starts go around and capture the points by shooting them. Once all the points are controlled by you (indicated by the points turning to your color) then finish the match and the achievement will unlock.
Important information from Xonatron:
The control points are visible as white on the map (before they are controlled) and spaced out like the dots on the "5" on a six-sided die (dice), at least in the first stage of the game. And put the time limit up high because as soon as you get all 5 control points, the game ends. No need to wait for the timer to run out.
