I had initially believed that this needed to be done just as you were dying to pop but it seems to be a bit trickier than that. When your health is down to the last portion of your health bar wait until an enemy is getting ready to attack you. Right before he attacks pop the flare and chase him off, burning away some of his shadow barrier in the process. This should pop the achievement.
Take note that this may have been a bit of an achievement glitch. The achievement may only require that you pop off a flare just before you die (i.e. while your screen is grey) but I've seen it not work that way a few times. Doing it this way will spare you any confusion although you're likely to get it without trying through the course of the game. |

| when the screen goes black and white use a HAND HELD FLARE and you will get this achievement i tried like 20 times with the flare gun until i realized you get the handheld flares in act 2 i believe. but this achievement only pops with a hand held falre. |

| This can be a little misleading, basically when you are being attacked wait until your health goes right down (screen goes black and white and health bar flashes) and pop a flare scaring off the Taken, cheev central! |

Simple - Let some of the enemies take a swing at you untill your screen turns gray/black and whitte, then use a flare when an enemy is about to take a swing at you, and keep holding it by holding the RB button.
Note - You may randomly unlock this when holding a flare. |

I got this achievement during the level where you meet the kidnapper on the way to Lover's Peak. As he takes your gun and leaves you with only flares it is definitely easier to get the reaction time right.
You need to wait until your health is almost completely depleted, with the heart beat and gray screen. Then ignite a flare just as a taken is in the process of attacking you (i.e. right as he is swinging, just before he hits you.) |

| When you have the least amount of life possible (one hit from anything and you're dead). Use a hand held flare when in range of any taken knocking them back to gain this achievement. |
