Complete chapter 2. You must guide all the companions through the Pulse Vestige to confront the Fal'Cie inside. As with the first chapter there are no major branching pathways, so all the items should be easy enough to find.

The boss is the Anima with the Right and Left Manipulators. Take out the manipulators to disable it's attacks and wail on it. The Anima will then regenerate the Manipulators, rinse and repeat. Heal as necessary with potions. A 5-star ranking is easy enough.
