This is a story related achievement from the Darkspawn Chronicles DLC and cannot be missed.

After fighting your way through Denerim, you'll end up back on the roof of Fort Drakon where the last Grey Warden along with some of their companions will be fighting the Archdemon. Simply defeat all the enemies whilst protecting the archdemon to finish the quest.

The achievement doesn't unlock until after the final cut scene.
