I was having trouble finding where some of the weapon upgrades were but then I found this guide and it proved to be very helpful. I hope it helps others who also had any trouble with this achievement.

| Along with finding upgrades throughout missions, you can purchase upgrades on Omega, The Citadel, Illium and Tutchanka. The in-game achievement tracker will let you know how many you have left on a specific weapon after upgrading it. Use those in the Tech Lab where Mordin is on the Normandy and the achievement will unlock when all upgrades are complete. |

here is a guide for finding the resources to research the upgrade but you will need the lair of the shadow broker dlc for this cheat.

scroll down to the Delivery Pickup section and the 2 note sections and read it and hopefully you should get enough of each resource to get the upgrades. |
