After speaking to Jacob two to three times after different missions, then you can follow through with his loyalty mission with positive remarks in conversation when he talks about the distress signal and his dad.

After visiting the Colony of Horizon Jacob ask to speak with you. When you do you will receive the quest "Jacob: The Gift of Greatness" when you complete the mission this achievement will unlock.
