Unless you decided to quit the game before any action starts then you will 100% definitely get this achievement. Just before you leave the Metro you will encounter your first glimpse of what creatures await you. These "rat/mole like" creatures are called Nosalises and are pretty easy to deal with. Three of these will attack you. Simply kill one and the "First Blood" achievement will pop up giving you a boost of 5 on your gamerscore.

Right when you start the game you have a bit of walking, to get the basic set up of the game. Once you arrive in the "upper levels" you will face 2 or 3 nosalises (basically giant mole rats). When you kill your first nosalise you should see this achievement pop up. Very easy.
