You are required to play through 93 areas online and without other human players, but AI teammates can still be used. Areas are the segments within each chapter of each episode. Farm levels for boxes and points by yourself and it will unlock eventually

EDIT: Make sure you have your connection type set to "Xbox Live" in the campaign menu. Fill the private slots so that nobody can join
