First off to get this you need to use the Plague Chapion. Beyond that you can use Heretics and any unit that can be given the mark of Nurgle which at the moment is just the Predator. You can also use Plague Marines, and the Great Unclean one.

You can not use Mark of Tzeentch or Khorne, Bloodletters or Blood Crushers

If you use a neutral unit, one without a mark and do not upgrade before the end of the game you will still get the achievement.

This one is harder to get legit than the others as you have a lot less options in Tier one however if you use you shrines well to heal your units it's not too hard. This one would be easier to get in a team game as your team mates can make up for your limited unit choice and you can support them with your shrines.

Just adding something to Hybridchild's solution:

Make sure you invest in your hero. The Plague Champion is an absolute terror against infantry with the tier 2 armor, grenades and sword, destroying and raising the enemy as undead allies.

It's better to spend requisition and power to build turrets to protect important victory points or power nodes than building plague marines.

"Rush" till tier 3 so you have access to the Chaos Predator, give him the mark of nurgle and you have a very, very, slow tank but with lots of HP. It's only good to protect important places, since you can't "kite" enemies being it so slow, so make sure you have worshippers repairing it.

(All this is mainly for Team Battle mode. I only play in this mode, so I can't confirm these tips will be of any good in 1 vs 1 or 2 vs 2)

There seems to be some disagreement about which units you can use. Generic units that can't recieve the mark of Nurgle but can get that of Khorne or Tzeentch don't void the achievement so long as you don't give them the other gods mark.
It's best to play team battle and hope your teammates play offensive heros. Unit variety is fine, you just cant upgrade the majority of them. Play defensively and spam the Plague champions turrets.

List of usable units:
Plague champion- must have
Heretics
Chaos SM- dont upgrade with either the mark of Khorne or Tzeentch
Havocs- again don't give them a mark (the weapons are marks)
Chaos Dreadnought- dont upgrade
Plague marines
Chaos Predator- Mark of Nurgle only
Great Unclean one

You can only win this using the Plague hero, your T1 worshippers worship your God so they don't count against you.

Building anything generic voids this Achievement, such as Chaos Marines or Chaos Dreadnoughts.

Plague commander can build the Plague Marines in T2 and the Big Nurgle Boss in T3, all other units are off limits.

The easiest thing to do is just pick the commander and then hope the other guy forfeits, that's how I got mine.
