Pretty simple to obtain, just use melee all the way through the level, as such ,mission 2 (the one where you box your dad) is best suited for this as majority of the level you are forced to use melee anyhow.

I thought I'd screwed up at one point when a take down fired the gun but the achievement still unlocked so that would suggest that only pressing RT will nullify the achievement.

Also difficulty doesn't matter so for easiest time set it to cadet although I got it on Detective on the level detailed above.
