The achievement requires you to open 10 doors using a hostage head. Basically, this means you have to open a door the way you normally would (with A button when prompted), however you have to be using an enemy as a 'human shield' when you go to open the door.
To take an enemy hostage, press A+X when near one, this will cause you to grab the guy, then you press X after you are grappling to take him hostage. At that point you can march him all over the place. Now you are ready to complete the challenge.
I did this in mission 2 - Shadowboxing on Cadet Mode.
Progress really far into the level. After Redwater and his goons shoot the guys you cornered you will have to holdout for 3 minutes or so just staying alive. After that, progress to the point where you come upon two guys talking and you are forced to listen to what they are saying. After that you will chase one of the guys. The chase will take you into an area with a lot of crates and a forklift. Kill all the guys in this area and move to the chain link fence and gate that will take you to the next area. A guy will spawn from the left and come towards you shooting. Let him kick the gate in and then take him hostage as described above.
There will be a large brown set of double doors ahead of you. Go to the doors, press A to open them with the hostage head, and then back away. The doors will close again. Repeat this 10 times and you have the achievement. It should pop in game after the 10th opening.

| This cheevo should have been called "Temple to the head" cause the way I did it was on mission 9 the old fashioned way, its right at the beginning of the level too just load that lvl make it to the police station with temple & progress to the 2nd computer monitor room & there some brown double doors there it also saves right before you get there so slam temple on the closed door & reload checkpoint again & again, should take less the 5 min to get the cheevo, hope that gets it. |

During Mission 9
After you Repel the attack from the South Wing, which is after you kill the 2 tanks. Head down the corridor and up the stairs heading towards the rooftop. On route there will be a set of double doors with "TRAINING" above the door. Grab a hostage (A+X,X) and use him to open the door. Step back from the door at it closes.Rinse and repeat untill the achievement unlocks.

For those of you who missed that and are playing the game through another area is in level 10.
After you get done covering the GCPD with the sniper rifle (after you blow the comm center) you will need to run down to the next mission area. If you head towards the stairs you will need to kill a couple soldiers and head through a scanner. To the left above the stairs there will be a mounted machine gun and several troops will come running to attack. Kill all but one of these men, take the last one hostage (X+A) and head past the stairs to a barrack. There are double doors. Open the double doors with his head 'A' and back up. The doors will close, move forward and open them again with 'A'. Do this until the achievement pops. |

You can do this on the first mission when you learn about disarms. Once you beat the enforcer and go up the elevator there's a guy waiting at the top, disarm him, clinch (A+X) then hit (X) and drag him the right side, left door hit (A) , reload check point. If you can do it fast enough you can get both doors in one pass.
