The easiest way to get this is with the two controller method. Make sure the team you use has a pretty good 3 point shooter. For the purposes of this achievement I used Kevin Durant.

Pass the ball to Durant and take him out to the three point line. With the second controller take control of the person guarding him and bring him close to Kevin but not right on top of him. This part is tricky as you have to start Kevin's shot and attempt an intentional foul with the player guarding him. A second after hitting the X button hit the back button on the second controller and while Durant is in the process of shooting he will be fouled and upon making the three pointer just swish the free throw and the cheevo will pop.
