In chapter 5, when you reach the bridge there is a mech with an explosion symbol above it. Damage it and then do a takedown. Once you have done that, you can then aim the mech and push a to have it explode at the enemy. Aim at the two other mechs near it (they should be closing in on you) and frag them for the achievement.
